How do I begin behavioral health services?

For anyone seeking out individual and/or family therapy sessions, you can simply reach out to Revival Therapy via our Referrals email ([email protected]) OR you can call Revival at 702-808-8141. You do NOT need a referral from a doctor to begin behavioral health/mental health services. If your child is under guardianship of DFS, please be sure to check in with the child’s caseworker for consent to move forward in this process. 

Where are behavioral health services taking place?

Behavioral/Mental Health services can take place in the home, in the office, or via Telehealth. This depends upon availability, locations, and insurance types. For more information about your specific options, please reach out to Revival Therapy ([email protected]).

How do I begin OT services?

Occupational Therapy requires a referral from your doctor or child’s pediatrician. You will need to make an appointment to be seen by the doctor, and request that they write a prescription (‘script’) for OT services. This can be faxed directly to Revival Therapy at 702-944-5498. If your child is under guardianship of DFS, please be sure to check in with the child’s caseworker for consent to move forward in this process.

How do I begin ABA services?

Prior to making appointments for referral, please reach out to Revival Therapy to obtain the ABA checklist and required documentation needed for your doctor’s visit.

ABA services require a referral from your doctor or child’s pediatrician. You will need to make an appointment to be seen by the doctor, and request that they write a prescription (‘script’) for ABA services. The doctor will also need to complete an FA11F form in order for Revival to schedule assessment/services. These can be faxed directly to Revival Therapy at 702-944-5498. If your child is under guardianship of DFS, please be sure to check in with the child’s caseworker for consent to move forward in this process.

How long are the waitlists?

Behavioral/Mental Health: depending upon staff scheduling this can be immediate scheduling to 6 weeks out, depending on location. Revival will keep you updated on scheduling options and availability.

OT Services: depending upon staff scheduling this can be immediate, though it is largely dependent upon location for in home services; North Las Vegas waitlist is currently up to 8 weeks before assessment can be scheduled. Scheduling assessments for services can only be completed once all required documentation is received. Revival will keep you updated on scheduling options and availability.

ABA Services: depending upon staff scheduling this can be immediate scheduling up to 2 months out. Scheduling assessments for services can only be completed once all required documentation is received. Revival will keep you updated on scheduling options and availability.

Can I request a specific therapist?

Yes! When reaching out to Revival for scheduling, you can always request a specific therapist. Let Revival know who you are interested in working with, and they can let you know the availability of that therapist at that time.

What is your cancellation policy?

Just as you are committed to the therapeutic process, so is Revival Therapy. Revival does their best to be flexible with clients and families while also recognizing the need for consistency in scheduling for the most effective therapeutic outcomes. Revival requires a minimum of 24 hours in advance notice of rescheduling for sessions outside of emergency incidents. You can talk to your therapist directly to receive a copy of the policy needed in consents for services.
